Thank you for your interest in our practice. We look forward to working with you. Here is some important information about your first visit with us.
Before You Arrive
Here is a link to our New Patient Intake Form. Please complete this form at least 1 day before your appointment.
If you have had X-rays and/or an MRI of any portion of your spine, it is very important that you either have the disc(s) and report(s) mailed to our office or drop them off at our front desk at least 2 days before your appointment to allow time for Dr. League to review them so that he has a clearer and more thorough understanding of your case before starting care.

We are here to provide you with the very best care and attention. At Cheshire Chiropractic Center, we understand that unanticipated events occur in everyone’s lives. However, out of respect for both our practitioners and the patients who are trying to rearrange their busy schedules, we ask that you do your very best not to cancel appointments last minute or fail to show up for your scheduled appointment. In our commitment to provide an outstanding experience to all of our patients and out of consideration for your practitioners’ time, we have adopted the following policy:
You may cancel or reschedule your appointment without charge if we have more than 24 hours notice. Out of consideration and respect of our practitioners’ time, if you cancel or reschedule with less than the aforementioned notice, you will be charged an $85 cancellation fee. Also if you do not show up for your scheduled appointment, you will be charged the $85 fee. We understand that sometimes life is a little out of your control and that unforeseen circumstances happen. Thank you for your understanding. We look forward to helping you reach your health goals.
Arrival at the Office
Please arrive for your appointment 15 minutes prior to the scheduled start time to allow time for:
- Viewing of a short video explaining chiropractic treatment. Click here if you would like to view ahead of time
- A 3-D laser scan of your feet to be completed. Click here to learn more
- Any remaining paperwork or copying of your insurance card
- Familiarizing yourself with our staff and the facility
Cost & Appointment Length
Between early arrival and the length of your first visit, the average time at our office is 45 – 60 minutes.
If you would like to find out ahead of time what your insurance benefits are, including your co-pay, you can call the member services number on the back of your insurance card and ask them what your chiropractic benefits are.
